Summary
NSAIDs such as parecoxib (via its active metabolite valdecoxib) can blunt the antihypertensive effect of valsartan and reduce renal function, particularly in older or volume-depleted patients. Hyperkalemia and acute kidney injury risk are also increased.
What this interaction means in plain words
Amlodipine-valsartan and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of COX-2-derived renal prostaglandins reduces renal blood flow and sodium excretion, which antagonizes ARB effect and impairs potassium and renal handling.
What to do
Monitor blood pressure, serum creatinine and potassium, and ensure adequate hydration. Use the shortest effective parecoxib course or choose an alternative analgesic in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
