Summary

Tricyclic-type antidepressants can potentiate the cardiovascular effects of beta-agonists such as salmeterol. This may cause tachycardia, palpitations, QT prolongation and arrhythmia, and salmeterol labeling advises caution within and shortly after tricyclic therapy.

What this interaction means in plain words

Amoxapine and Fluticasone-salmeterol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive QT-prolonging and adrenergic effects, with tricyclic noradrenaline reuptake inhibition enhancing the vascular response to beta-agonists.

What to do

Use with ECG and heart rate monitoring and consider an alternative to the tricyclic or the LABA in patients with cardiac risk. Check potassium in at-risk patients.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
