Summary
Combining an SSRI with a tricyclic-type antidepressant increases the risk of serotonin toxicity. Paroxetine is a strong CYP2D6 inhibitor and can raise amoxapine concentrations, increasing anticholinergic, sedative and cardiac effects.
What this interaction means in plain words
Amoxapine and Paroxetine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive serotonergic effects and paroxetine inhibition of CYP2D6-mediated amoxapine metabolism.
What to do
Avoid if possible. If coadministered, use low amoxapine doses with slow titration and monitor for serotonin syndrome, anticholinergic effects and ECG changes.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
