Summary

Amoxapine blocks reuptake of norepinephrine and serotonin, and tranylcypromine is an irreversible nonselective MAOI. Together they can cause serotonin syndrome or hypertensive crisis.

What this interaction means in plain words

Amoxapine and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

Reuptake inhibition combined with irreversible MAO inhibition increases synaptic serotonin and norepinephrine.

What to do

Do not combine. Observe a washout of at least 14 days after stopping tranylcypromine before starting amoxapine, and an adequate washout of amoxapine before starting an MAOI.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
