Summary
Polymyxin B and amphotericin B deoxycholate are both nephrotoxic, and polymyxin B can be ototoxic. With topical triple antibiotic products, systemic absorption is usually minimal on intact skin, but can be significant with large wounds, burns or irrigation. In such settings combined renal injury is possible.
What this interaction means in plain words
Amphotericin b deoxycholate and Bacitracin-neomycin-polymyxin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive nephrotoxicity through direct renal tubular injury by polymyxin and amphotericin B.
What to do
For topical use on small areas of intact skin no action is needed. When large-area or irrigation use occurs, monitor serum creatinine and electrolytes and consider a non-nephrotoxic alternative.
