Summary
Arformoterol, a long-acting beta-2 agonist, can lower serum potassium by driving potassium into cells. Combined with the potassium-wasting effect of hydrochlorothiazide, hypokalemia may be more pronounced and could promote arrhythmias, especially in patients with hypoxia or heart disease. Valsartan partly offsets this by retaining potassium.
What this interaction means in plain words
Arformoterol and Hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Arformoterol belongs to Respiratory; Hydrochlorothiazide-valsartan belongs to Cardiovascular.
Why it happens
Additive hypokalemia: beta-2 stimulation shifts potassium intracellularly while thiazides increase renal potassium excretion.
What to do
Check serum potassium and magnesium at baseline and periodically, particularly in those with cardiac disease or taking other potassium-lowering drugs. Replace potassium or magnesium if low.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
