Summary

Methadone prolongs the QT interval, and long-acting beta-2 agonists such as arformoterol can cause tachycardia, hypokalemia and QT prolongation. Together they may increase the risk of ventricular arrhythmias, particularly in patients with cardiac disease.

What this interaction means in plain words

Arformoterol and Methadone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Arformoterol belongs to Respiratory; Methadone belongs to Musculoskeletal.

Evidence level: Low

Why it happens

Additive QT-prolonging and hypokalemic effects of methadone and the beta-2 agonist.

What to do

Monitor ECG and serum potassium in patients with additional QT risk factors, and avoid exceeding recommended arformoterol doses.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
