Summary

Corticosteroids given with non-depolarizing neuromuscular blockers, particularly during prolonged ICU use, have been associated with acute quadriplegic myopathy and prolonged muscle weakness. This can delay recovery and weaning from ventilation.

What this interaction means in plain words

Atracurium and Betamethasone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Atracurium belongs to Musculoskeletal; Betamethasone belongs to Endocrine.

Evidence level: Moderate

Why it happens

Combined effects on muscle fiber integrity and neuromuscular function leading to myopathy; the exact mechanism is not fully established.

What to do

Keep the duration of neuromuscular blockade and steroid exposure as short as possible and monitor neuromuscular function. Assess for persistent weakness and consider creatine kinase if myopathy is suspected.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
