Summary

Concurrent corticosteroid and fluoroquinolone use increases the risk of tendinitis and tendon rupture, most often of the Achilles tendon. The risk is higher in older patients and with prolonged or high-dose steroid exposure.

What this interaction means in plain words

Betamethasone and Ofloxacin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Betamethasone belongs to Endocrine; Ofloxacin belongs to Anti Bacterial.

Evidence level: High

Why it happens

Both drug classes impair tendon collagen turnover and tenocyte function, producing additive tendon toxicity.

What to do

Avoid the combination where possible, especially in patients over 60 or with renal impairment. If used, advise the patient to stop the quinolone and rest at the first sign of tendon pain or swelling.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
