Summary
Aminoglycosides and polymyxins can impair neuromuscular transmission and may add to the effect of botulinum toxin. This could cause excessive or spread-out muscle weakness. With topical use, systemic absorption is usually small, so the risk is mainly theoretical unless the preparation is applied to large or broken skin areas.
What this interaction means in plain words
Bacitracin-neomycin-polymyxin and Incobotulinumtoxina have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Additive impairment of neuromuscular transmission, with aminoglycoside and polymyxin presynaptic blockade of acetylcholine release.
What to do
For topical use on limited intact skin, no action is needed beyond awareness. If systemic or extensive use is required, monitor for excessive weakness, swallowing or breathing difficulty after the toxin injection.
