Summary
Polymyxin B and neomycin can potentiate rocuronium's neuromuscular blockade. This may prolong paralysis and delay recovery of spontaneous breathing. The risk is mainly with systemic or irrigation use, not small topical areas.
What this interaction means in plain words
Bacitracin-neomycin-polymyxin and Rocuronium can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Bacitracin-neomycin-polymyxin belongs to Anti Bacterial; Rocuronium belongs to Musculoskeletal.
Why it happens
Additive pharmacodynamic neuromuscular blockade through reduced presynaptic acetylcholine release and postsynaptic receptor effects.
What to do
Avoid systemic, irrigation or large-area combination antibiotic exposure during neuromuscular blockade where possible. If used, monitor neuromuscular function with train-of-four and ensure ventilatory support is available.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
