Summary

Cyclosporine may increase exposure to the estrogen and bazedoxifene components by reducing their intestinal and hepatic metabolism and efflux. This could increase estrogenic adverse effects such as breast tenderness, fluid retention and thromboembolic risk. The clinical effect is generally modest.

What this interaction means in plain words

Bazedoxifene / conjugated estrogens and Cyclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Cyclosporine inhibits CYP3A4 and P-glycoprotein, reducing first-pass metabolism and efflux of the components.

What to do

Monitor for estrogen-related adverse effects and adjust the estrogen dose if they occur. Also monitor cyclosporine levels, since estrogens can alter them.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
