Summary

Fludrocortisone increases renal potassium excretion and thiazide diuretics do the same, so combined use can produce significant hypokalemia. Hypokalemia may cause arrhythmias and muscle weakness, particularly in patients on digoxin or with heart disease.

What this interaction means in plain words

Bendroflumethiazide and Fludrocortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive kaliuresis from mineralocorticoid activity and increased distal sodium delivery caused by thiazide diuresis.

What to do

Check serum potassium and blood pressure periodically and supplement potassium or adjust doses if levels fall.
