Summary

Thiazide diuretics can raise blood glucose and may reduce the glucose-lowering effect of sulfonylureas such as gliclazide. Loss of glycaemic control is more likely at higher thiazide doses or in patients with marginal control.

What this interaction means in plain words

Bendroflumethiazide and Gliclazide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Moderate

Why it happens

Thiazides cause hypokalaemia and reduced insulin secretion and may impair peripheral insulin sensitivity, opposing the sulfonylurea effect.

What to do

Monitor blood glucose and HbA1c when starting or adjusting the thiazide, and increase the antidiabetic dose if control deteriorates. Check potassium and correct hypokalaemia.
