Summary
Thiazide diuretics can raise blood glucose by impairing insulin secretion and reducing insulin sensitivity, partly through potassium loss. This may reduce the glycemic control achieved with glyburide and metformin, particularly at higher thiazide doses.
What this interaction means in plain words
Bendroflumethiazide and Glyburide-metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Pharmacodynamic antagonism through thiazide-induced hypokalemia, reduced insulin release and increased insulin resistance.
What to do
Monitor blood glucose and HbA1c after starting or changing the thiazide dose and keep potassium in the normal range. Adjust the antidiabetic regimen if control worsens.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
