Summary

The cytotoxic component of brentuximab vedotin, MMAE, is metabolized by CYP3A4, so CYP3A4 inhibitors can raise its exposure and the risk of neutropenia. Isoniazid is only a weak inhibitor, so any effect is likely modest. Isoniazid also adds neuropathy and hepatotoxicity risk.

What this interaction means in plain words

Brentuximab vedotin and Isoniazid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Brentuximab vedotin belongs to Oncology; Isoniazid belongs to Anti Bacterial.

Evidence level: Low

Why it happens

Possible weak CYP3A4 inhibition by isoniazid increasing MMAE exposure, with additive neurotoxicity.

What to do

Monitor blood counts for neutropenia and for peripheral neuropathy. Give pyridoxine as per isoniazid practice.
