Summary
Primidone, which is partly metabolized to phenobarbital, adds CNS depression to buprenorphine. It also induces CYP3A4 and may reduce buprenorphine concentrations, risking withdrawal or reduced analgesia.
What this interaction means in plain words
Buprenorphine and Primidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Buprenorphine belongs to Musculoskeletal; Primidone belongs to Central Nervous System.
Why it happens
Additive CNS depression plus CYP3A4 induction by primidone and its phenobarbital metabolite.
What to do
Monitor sedation, respiration and withdrawal symptoms, and adjust buprenorphine to clinical response. Reassess dosing if primidone is started, changed or stopped.
What to ask your pharmacist
