Summary

Combining morphine with long-acting buprenorphine injection adds opioid-related sedation and respiratory depression. Buprenorphine's high receptor affinity and prolonged presence can also partly block morphine's analgesia, making pain control unpredictable and overdose risk difficult to manage.

What this interaction means in plain words

Buprenorphine long-acting and Morphine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive mu-opioid CNS and respiratory depression, with competitive receptor binding by buprenorphine, a partial agonist with high affinity.

What to do

Avoid the combination where alternatives exist; if both are needed, use the lowest effective morphine dose with monitoring of sedation and respiratory rate. Ensure naloxone is accessible.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
