Summary
Opioids given with MAO inhibitors can cause severe reactions, including serotonin syndrome-like excitation, hyperpyrexia, hypertension or hypotension, and coma. Labeling of nonselective MAOIs advises against co-use with opioids.
What this interaction means in plain words
Isocarboxazid and Morphine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Isocarboxazid belongs to Central Nervous System; Morphine belongs to Musculoskeletal.
Why it happens
Nonselective MAO inhibition combined with opioid effects leads to serotonergic toxicity and unpredictable CNS and cardiovascular responses.
What to do
Avoid concurrent use and allow about 14 days after stopping isocarboxazid before giving morphine where possible. Use a non-opioid alternative, and if an opioid is unavoidable, monitor intensively.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
