Summary

Barbiturates and benzodiazepines together produce additive sedation and respiratory depression, which can be dangerous in overdose or in vulnerable patients. Butabarbital may also induce hepatic enzymes and modestly lower clonazepam exposure.

What this interaction means in plain words

Butabarbital and Clonazepam can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive GABA-A mediated CNS depression, with possible CYP3A4 induction by the barbiturate.

What to do

Avoid combining where possible; if needed use reduced doses and monitor sedation and respiration. Reassess seizure control if clonazepam is being used as an anticonvulsant.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
