For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Barbiturates induce CYP2B6 and CYP3A4, which can speed ifosfamide conversion both to active alkylating metabolites and to neurotoxic chloroacetaldehyde. This may increase toxicity, including encephalopathy and nephrotoxicity, and the sedative effect of the barbiturate may also confuse the assessment of neurotoxicity.

What this interaction means in plain words

Butabarbital and Ifosfamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Induction of CYP2B6 and CYP3A4 by butabarbital increases ifosfamide bioactivation and toxic metabolite formation.

What to do

Prefer a non-inducing alternative where possible. If combined, monitor for neurotoxicity, renal function and myelosuppression and adjust the ifosfamide dose as needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
