Summary

Both drugs depress the central nervous system, so combining them can produce marked drowsiness, dizziness and slowed psychomotor performance. Clozapine can also cause orthostatic hypotension and, rarely, respiratory depression, and an opioid may add to these effects.

What this interaction means in plain words

Butorphanol and Clozapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Butorphanol belongs to Musculoskeletal; Clozapine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive CNS depression from opioid agonist-antagonist activity and clozapine's antihistaminic, anticholinergic and antiadrenergic effects.

What to do

Start butorphanol at a reduced dose and titrate slowly, monitoring sedation, blood pressure and respiratory rate. Warn patients against driving or operating machinery.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
