Summary

Selegiline at antidepressant doses causes nonselective MAO inhibition, and combining it with sympathomimetic stimulants such as caffeine may provoke hypertensive reactions. Palpitations, headache and tachycardia may occur, and severe hypertension is possible with high caffeine intake.

What this interaction means in plain words

Caffeine and Selegiline transdermal can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Low

Why it happens

MAO inhibition combined with caffeine-induced catecholamine release produces additive pressor effects.

What to do

Avoid caffeine-containing medicines and limit dietary caffeine in patients on transdermal selegiline. Monitor blood pressure and counsel on symptoms of hypertensive crisis.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
