Summary

Intravenous calcium can reverse the hemodynamic effects of calcium channel blockers, which is used therapeutically in overdose but may reduce the effect of nifedipine if given unintentionally. Hypercalcemia from calcium administration can blunt blood pressure lowering.

What this interaction means in plain words

Calcium chloride and Nifedipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Elevated extracellular calcium competes with channel blockade, increasing calcium influx into vascular and cardiac cells.

What to do

Monitor blood pressure and heart rate when calcium chloride is given to a patient taking nifedipine, and avoid unnecessary calcium dosing. Use deliberately only for calcium channel blocker toxicity.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
