Summary

Cannabidiol can inhibit and possibly induce CYP1A2 and other enzymes, which may change pimozide exposure. Pimozide is mainly cleared by CYP3A4 and CYP2D6 with a minor CYP1A2 contribution, and cannabidiol may also inhibit CYP2D6 and CYP3A4, so levels and QT risk could rise.

What this interaction means in plain words

Cannabidiol and Pimozide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Cannabidiol belongs to Central Nervous System; Pimozide belongs to Central Nervous System.

Evidence level: Low

Why it happens

Cannabidiol inhibition of CYP enzymes (including CYP1A2, CYP3A4 and CYP2D6) that metabolize pimozide.

What to do

Monitor for pimozide adverse effects such as sedation, extrapyramidal symptoms and QT prolongation. Consider an ECG and a pimozide dose reduction if cannabidiol is started.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
