Summary

Cedazuridine inhibits cytidine deaminase, which is not the main pathway for capecitabine activation or clearance, which relies on carboxylesterase, thymidine phosphorylase and DPD. An effect on capecitabine exposure is possible for its intermediate 5'-deoxy-5-fluorocytidine, which is deaminated by CDA, but the clinical impact is uncertain.

What this interaction means in plain words

Capecitabine and Cedazuridine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Inhibition of cytidine deaminase may alter conversion of the capecitabine intermediate 5'-DFCR to 5'-DFUR, with unestablished clinical effect.

What to do

Avoid routine combination; if both are needed, monitor for fluoropyrimidine toxicity such as diarrhea, hand-foot syndrome and myelosuppression.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
