Both drugs are toxic to the kidneys and the inner ear, and their combination raises the risk of acute kidney injury, hearing loss and vestibular damage. Capreomycin can also cause neuromuscular blockade, which may be additive with the aminoglycoside. Damage to hearing and balance can be permanent.
What this interaction means in plain words
Capreomycin and Streptomycin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive nephrotoxicity, ototoxicity and neuromuscular blocking effects of two drugs with overlapping toxicity profiles.
What to do
Avoid the combination where an alternative regimen exists. If unavoidable, monitor serum creatinine, electrolytes (potassium, magnesium, calcium) and audiometry regularly and adjust doses to renal function.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
