Drug interactions
captopril + ibuprofen
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
NSAIDs may blunt captopril’s antihypertensive effect and increase risk of acute kidney injury and hyperkalemia, especially in older adults, CKD, HF, dehydration, or with high-dose/chronic NSAID use.
Mechanism of action
NSAID prostaglandin inhibition → afferent arteriolar vasoconstriction and Na/H2O retention; ACE inhibitor causes efferent dilation → reduced GFR and impaired K+ excretion; attenuated vasodilatory/prostaglandin effects on BP.
Clinical management
Avoid chronic/high-dose NSAIDs; consider acetaminophen or topical NSAIDs. If needed, use lowest dose/shortest duration. Check BP, SCr/eGFR, and K+ at baseline and within 1–2 weeks after initiation or dose change; monitor periodically.
