Summary

Carbenoxolone has mineralocorticoid-like activity that promotes potassium loss and sodium retention. Salmeterol can also lower serum potassium by driving potassium into cells, so combined use may produce clinically relevant hypokalemia, particularly at higher beta-agonist doses or in patients with other risk factors. Hypokalemia can predispose to arrhythmias.

What this interaction means in plain words

Carbenoxolone and Fluticasone-salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive hypokalemia: renal potassium wasting from carbenoxolone plus beta-2-mediated intracellular potassium shift from salmeterol.

What to do

Monitor serum potassium, especially at treatment start and in patients with cardiac disease. Replace potassium as needed and consider stopping carbenoxolone if hypokalemia develops.
