Summary

Pyridoxine is a cofactor for dopa decarboxylase and can speed the peripheral conversion of levodopa to dopamine when levodopa is given alone. This lowers levodopa delivery to the brain and may reduce efficacy. Carbidopa blocks this effect, so it is of little relevance with carbidopa-levodopa combinations.

What this interaction means in plain words

Carbidopa-levodopa and Pyridoxine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Moderate

Why it happens

Pyridoxine enhances peripheral dopa decarboxylase activity, which carbidopa inhibits.

What to do

No routine action is needed with carbidopa-levodopa at usual vitamin doses. Monitor motor response if high-dose pyridoxine is used.
