Summary

Some antibiotics, including cephalosporins, have been associated with lower mycophenolic acid exposure, probably by reducing gut bacteria that deconjugate the glucuronide and allow its reabsorption. The legacy renal competition explanation is not well supported. Reduced exposure could weaken immunosuppression.

What this interaction means in plain words

Cefadroxil and Mycophenolate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Probable reduction of enterohepatic recirculation of mycophenolic acid glucuronide due to gut flora changes.

What to do

Watch for signs of graft rejection or reduced immunosuppression during and after the antibiotic course. Consider mycophenolic acid level monitoring where available.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
