Summary
Oral neomycin is poorly absorbed, but systemic absorption can occur with impaired gut mucosa or renal impairment, and it is nephrotoxic and ototoxic. Combined with cephalosporins, additive kidney injury has been suggested. Cefadroxil itself is only weakly nephrotoxic.
What this interaction means in plain words
Cefadroxil and Neomycin po can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Potential additive nephrotoxicity from systemically absorbed neomycin with reduced renal clearance of both drugs.
What to do
Monitor renal function, especially with renal impairment or prolonged neomycin use. Adjust cefadroxil dose to creatinine clearance.
