Summary

Chlorambucil suppresses bone marrow and lymphoid function, which can blunt the antibody response to cholera vaccine. Protection may therefore be incomplete in treated patients. The vaccine is oral and live-attenuated in some formulations, so replication in immunosuppressed patients is also a theoretical concern.

What this interaction means in plain words

Chlorambucil and Cholera vaccine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Cytotoxic immunosuppression by the alkylating agent reduces the vaccine-induced immune response.

What to do

Where possible, vaccinate at least 2 weeks before starting chlorambucil, and do not rely on the vaccine alone for protection. Reassess immunity or revaccinate after immune recovery, and reinforce food and water precautions.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
