Summary

Vaginally applied miconazole can be absorbed to a limited degree and may raise exposure to sulfonylureas. Reported cases with other miconazole forms involve enhanced glucose lowering and hypoglycemia. The risk with vaginal use is lower but is relevant in patients on long-acting chlorpropamide.

What this interaction means in plain words

Chlorpropamide and Miconazole vaginal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Miconazole inhibits CYP2C9, the main enzyme that metabolizes chlorpropamide, which can raise its plasma levels.

What to do

Monitor blood glucose and counsel the patient on hypoglycemia symptoms during and after the antifungal course. Reduce the chlorpropamide dose if glucose falls.

What to ask your pharmacist
