Summary

Thiazide-like diuretics can impair glucose tolerance and may reduce the effect of sulfonylureas. Hyponatremia can also add to chlorpropamide's SIADH-like effects.

What this interaction means in plain words

Chlorpropamide and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Diuretic-induced hypokalemia and reduced insulin secretion cause hyperglycemia, and hyponatremia is additive with chlorpropamide.

What to do

Monitor blood glucose and serum sodium and potassium. Adjust antidiabetic therapy if glucose control worsens.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
