Summary
Trilostane suppresses adrenal steroid synthesis including aldosterone, which can reduce potassium loss caused by chlorthalidone. It may also increase sodium loss and the risk of hypotension or hyperkalemia, especially with renal impairment.
What this interaction means in plain words
Chlorthalidone and Trilostane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of 3-beta-hydroxysteroid dehydrogenase reduces aldosterone synthesis, adding to the natriuresis of the thiazide-type diuretic while sparing potassium.
What to do
Monitor serum potassium, sodium and blood pressure during combined use, and avoid concurrent potassium supplements or potassium-sparing agents unless guided by levels.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
