Summary

Both drugs target complement C5, but they bind different epitopes. During a switch, the two antibodies and C5 can form drug-target-drug complexes, which may trigger Type III hypersensitivity reactions such as fever, rash, arthralgia or other immune-complex manifestations.

What this interaction means in plain words

Crovalimab and Eculizumab can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Crovalimab belongs to Miscellaneous; Eculizumab belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Simultaneous presence of two anti-C5 antibodies with different epitopes forms circulating immune complexes with C5.

What to do

Follow the labeled switching schedule so that the dose timing is coordinated with the last eculizumab dose. Monitor closely for immune-complex reactions after the switch and manage them promptly.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
