Summary

Iodinated contrast can cause acute kidney injury, and metformin accumulation in that setting raises the risk of lactic acidosis. Ethiodized oil is given intralymphatically or intra-arterially, and systemic exposure can occur, so the risk applies mainly in renal impairment.

What this interaction means in plain words

Dapagliflozin-metformin and Ethiodized oil can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Low

Why it happens

Contrast-induced decline in renal function reduces metformin elimination, leading to accumulation and lactic acidosis.

What to do

Hold metformin around the procedure in patients with eGFR below 30, acute kidney injury, or intra-arterial administration. Re-check renal function about 48 hours later before resuming metformin.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
