Summary
Imipramine has strong anticholinergic activity and is metabolised by CYP2D6, which darifenacin inhibits. Combined use increases the risk of anticholinergic toxicity (urinary retention, constipation, confusion, tachycardia) and may raise tricyclic concentrations.
What this interaction means in plain words
Darifenacin and Imipramine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Darifenacin belongs to Genitourinary; Imipramine belongs to Central Nervous System.
Why it happens
Additive antimuscarinic effects and CYP2D6 inhibition by darifenacin reducing imipramine clearance.
What to do
Monitor for anticholinergic effects and tricyclic toxicity, including ECG if the dose is high. Consider a lower imipramine dose or a non-antimuscarinic bladder agent.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
