Summary

Fosamprenavir (as amprenavir) inhibits CYP3A4, particularly when boosted with ritonavir, and increases exposure to the active metabolite of deflazacort. Corticosteroid adverse effects such as adrenal suppression, hyperglycemia and infection may be increased.

What this interaction means in plain words

Deflazacort and Fosamprenavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: High

Why it happens

Inhibition of CYP3A4 reduces clearance of deflazacort's active metabolite.

What to do

Give one-third of the usual deflazacort dose during coadministration. Monitor for glucocorticoid toxicity.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
