Summary
Parecoxib, a COX-2 selective NSAID prodrug of valdecoxib, taken with a corticosteroid can increase the likelihood of gastrointestinal ulceration and bleeding. COX-2 selectivity reduces but does not remove this risk, especially in older patients or those with prior ulcers.
What this interaction means in plain words
Deflazacort and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive gastrointestinal mucosal injury from corticosteroid effects and prostaglandin inhibition by the NSAID.
What to do
Use the shortest possible course of parecoxib, and consider a proton pump inhibitor in patients at higher GI risk. Watch for dyspepsia, melena and other signs of GI bleeding.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
