Summary

Dengue vaccine (Dengvaxia) is a live attenuated vaccine. Fludarabine-induced immunosuppression may blunt the immune response to it, and in a severely immunocompromised patient the live virus could cause vaccine-related disease.

What this interaction means in plain words

Dengue vaccine and Fludarabine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Fludarabine-induced lymphocyte depletion reduces the immune response to the vaccine and may allow uncontrolled replication of the live vaccine virus.

What to do

Do not give live dengue vaccine during fludarabine therapy or while the patient remains significantly immunosuppressed afterward; vaccinate once immune recovery is confirmed.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
