Summary

Dengue vaccine is a live attenuated vaccine, and immunosuppressive doses of corticosteroids may blunt the immune response and increase the risk of vaccine-related infection. Protection may therefore be inadequate.

What this interaction means in plain words

Dengue vaccine and Prednisolone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Glucocorticoid immunosuppression reduces vaccine response and may permit uncontrolled replication of a live attenuated virus.

What to do

Avoid live vaccine in patients on immunosuppressive steroid doses; defer vaccination until steroids are stopped for the interval stated in the product labeling. Low-dose or short-course use may be assessed individually.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
