What this interaction means in plain words

Desvenlafaxine and Metoclopramide intranasal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Central dopamine D2 blockade combined with serotonergic activity produces additive neurological toxicity; the mechanism is not fully established.

What to do

Use the lowest effective metoclopramide dose for the shortest time and monitor for tremor, rigidity, akathisia, hyperthermia and mental status change. Consider an alternative antiemetic if symptoms appear.
