Summary

Desvenlafaxine is a weak CYP2D6 inhibitor, which at higher doses may modestly raise concentrations of CYP2D6 substrates. Trifluoperazine is partly metabolized by CYP2D6, so extrapyramidal effects, sedation or QT effects could increase. The effect is usually small at standard desvenlafaxine doses.

What this interaction means in plain words

Desvenlafaxine and Trifluoperazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Weak inhibition of CYP2D6 by desvenlafaxine may reduce the metabolism of trifluoperazine.

What to do

Monitor for extrapyramidal symptoms, sedation and orthostasis. Consider a lower trifluoperazine dose if adverse effects appear, particularly with high desvenlafaxine doses.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
