Summary

Paclitaxel is metabolised mainly by CYP2C8 and to a lesser extent CYP3A4. Dexamethasone is not a recognised CYP2C8 inhibitor and, if anything, is a weak CYP3A4 inducer, so a clinically meaningful increase in paclitaxel exposure is not expected. Dexamethasone is routinely used as premedication in paclitaxel regimens.

What this interaction means in plain words

Dexamethasone and Paclitaxel protein bound have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

No established pharmacokinetic mechanism; dexamethasone may weakly induce CYP3A4, which has a minor role in paclitaxel clearance.

What to do

No dose adjustment is required; monitor for usual paclitaxel toxicities such as neutropenia and neuropathy.
