For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Rilpivirine is a CYP3A4 substrate, and repeated dexamethasone dosing may reduce its concentrations, raising the risk of virologic failure and NNRTI resistance. Single-dose dexamethasone is not a concern.

What this interaction means in plain words

Dexamethasone and Rilpivirine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Dexamethasone belongs to Endocrine; Rilpivirine belongs to Anti Virals.

Evidence level: High

Why it happens

Dexamethasone induces CYP3A4, increasing rilpivirine clearance.

What to do

Do not use more than a single dose of systemic dexamethasone with rilpivirine; select an alternative corticosteroid or antiretroviral for longer courses.
