Summary

Foscarnet and tobramycin are both nephrotoxic, and the aminoglycoside also carries ototoxic risk. Using both can cause additive kidney injury, electrolyte disturbance and renal impairment. Systemic exposure from a dexamethasone-tobramycin topical or ophthalmic product is low, but systemic tobramycin would raise the risk considerably.

What this interaction means in plain words

Dexamethasone-tobramycin and Foscarnet can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dexamethasone-tobramycin belongs to Anti Bacterial; Foscarnet belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Additive nephrotoxicity from two agents that each damage renal tubular cells.

What to do

If systemic tobramycin is used, monitor serum creatinine and electrolytes (calcium, magnesium, potassium) closely and keep hydration adequate. Avoid the combination where alternatives exist.
