Summary

Topiramate and diazepam both cause CNS depression, so combined use can increase drowsiness, dizziness and cognitive slowing. Topiramate is a weak CYP3A4 inducer at higher doses, but a meaningful effect on diazepam levels is uncommon.

What this interaction means in plain words

Diazepam and Topiramate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Diazepam belongs to Musculoskeletal; Topiramate belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive CNS depression, with a minor possible induction of diazepam metabolism by topiramate.

What to do

Titrate both drugs slowly and monitor for sedation and cognitive effects. Warn patients against driving until the combined effect is known.

What to ask your pharmacist

  • Should I take them at different times of day?
