Summary

Dichlorphenamide commonly causes potassium loss through its carbonic anhydrase inhibition. Cyanocobalamin can lower potassium when treating severe megaloblastic anemia because of rapid cell production, so low potassium is possible when both are used. This is mainly relevant at initiation of B12 therapy in severely deficient patients.

What this interaction means in plain words

Dichlorphenamide and Vitamin B12 can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dichlorphenamide belongs to Miscellaneous; Vitamin B12 belongs to Gastrointestinal Tract.

Evidence level: Low

Why it happens

Additive hypokalemia: renal potassium wasting from carbonic anhydrase inhibition plus increased cellular potassium uptake during hematopoietic recovery after B12 replacement.

What to do

Check serum potassium before and during early B12 repletion in patients on dichlorphenamide. Replace potassium as needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
